London

Monica Iofciu, Occupational Therapist and Catastrophic Case Manager at Harrison Associates

Biography

Monica Iofciu is an Occupational Therapist and Catastrophic Case Manager with extensive experience across NHS acute services, Adult Social Care and independent practice. Her work is centred on the management of complex adult cases, delivering structured, defensible and outcome-focused rehabilitation and care coordination for individuals with catastrophic and long-term conditions.

Monica specialises in complex neurological rehabilitation, supporting individuals with acquired brain injury, stroke, Parkinson’s disease, multiple sclerosis, motor neurone disease, cerebral palsy and dementia. She has significant experience managing cases with co-existing mental health conditions, including anxiety, depression, PTSD, personality disorders and schizophrenia, requiring coordinated multidisciplinary intervention and proportionate risk management.

Her practice is grounded in detailed functional and cognitive assessment, with particular strength in working with individuals presenting with fluctuating capacity, safeguarding concerns and high falls risk. She is experienced in contributing to Best Interest decision-making, developing and optimising care packages, and ensuring that rehabilitation and support plans are both clinically robust and sustainable within complex home environments.

Monica brings advanced expertise in complex moving and handling, including single-handed care and review of double-handed care packages, alongside specialist equipment provision and major housing adaptations. Her work includes close coordination with multidisciplinary teams across health, social care and housing systems, ensuring safe discharge, continuity of care and effective long-term planning.

In her current Deputy Team Manager role within a complex Adult Social Care Occupational Therapy service, Monica provides clinical oversight, manages safeguarding and escalation processes, and signs off major adaptation and housing reports. She has contributed to service improvement initiatives focused on delivering proportionate, cost-effective and defensible care planning, reinforcing her structured and governance-led approach to case management.

Her practice is underpinned by strong clinical reasoning, leadership and a clear understanding of statutory frameworks, ensuring delivery of coordinated, outcome-focused rehabilitation for clients with complex and catastrophic needs.
