Biography — Case Management Sharon Neave is a Catastrophic Case Manager and Occupational Therapist with nearly a decade of experience in neurological rehabilitation across major trauma centres, neurosurgical wards and specialist neurorehabilitation units. She specialises in adult traumatic brain injury, spinal cord injury and complex neurological presentations, including complete and incomplete spinal cord injuries. Her practice is centred on holistic, functional rehabilitation and the structured transition from hospital to home. Sharon has extensive experience coordinating complex care packages, including 24-hour support, respite provision and structured care calls. She works closely with statutory services and multidisciplinary teams to ensure rehabilitation pathways are realistic, sustainable and outcome-focused. Her leadership includes managing teams of consultants, neurologists, psychologists, physiotherapists, occupational therapists and specialist nursing staff, maintaining clear communication across professionals, families and external agencies. Sharon takes a holistic approach to paediatric case management, considering the child’s rehabilitation alongside their development, independence, family circumstances and longer-term needs. She works closely with families, treating clinicians, therapists, education and other professionals to coordinate realistic rehabilitation plans and maintain clear communication across the multidisciplinary team. Her wider experience in complex case management includes coordinating substantial care packages, specialist support and transitions between hospital, rehabilitation services and home.
