Eleanor Crowther, Expert Witness

Biography

Eleanor Crowther is a Registered Mental Health Nurse and Expert Witness with over 12 years’ post-registration experience, specialising in acute inpatient mental health care, clinical governance and breach-of-duty assessment. She has held senior clinical and leadership positions including Ward Manager, Lead Nurse and Clinical Services Manager, with extensive experience in patient safety, risk management and the investigation of adverse clinical events.

Her clinical expertise encompasses the assessment and management of patients presenting with acute psychosis, severe mood disorders, personality disorders, complex trauma, suicidal crises, self-harm and behavioural disturbance. She has substantial experience reviewing incidents involving observation failures, medication errors, safeguarding concerns, restrictive interventions, absconding and deterioration in mental state.

Eleanor provides independent, evidence-based opinion on mental health nursing standards, including risk assessment, care planning, clinical observations, recognition and escalation of deterioration, safeguarding and the management of vulnerable psychiatric inpatients. Her approach involves detailed review of clinical records, reconstruction of care timelines and assessment of whether nursing practice met the professional standards expected at the relevant time.

Her governance and investigative experience includes Root Cause Analysis, human factors reviews and consideration of individual clinical errors alongside wider organisational or systemic failings. She assesses care against the NMC Code, NICE guidance, the Mental Health Act, Mental Capacity Act and applicable local policies.

Eleanor holds a BSc in Mental Health Nursing, a Postgraduate Diploma in Mental Health Law and a qualification in Independent and Supplementary Prescribing (V300). She is registered with the Nursing and Midwifery Council and is undertaking the Trainee Approved Clinician (MPARC) pathway.
